Roche Recalls some ACCU-CHEK Aviva Glucose Meters
January 2006 (US), November 2005 (Canada): Some ACCU-CHEK Aviva Blood Sugar Meters have been voluntarily recalled by Roche Diagnostics. The affected blood sugar
monitors may give erroneous results or shut down completely. Should your meter
be affected, Roche will replace it. See information below.
The recall includes Aviva blood sugar monitors with U.S. serial numbers
52500000000 through 52510999999. Please call 1-888-591-5084 or visit www.accu-chek.com.

IN Canada the recall incluces any Aviva blood sugar meter with a serial number less than 526 10
999 999. Please call 1-800-363-7949 or visit www.accu-chek.com
for more information as these monitors may be affected.
Outside the U.S. and Canada the meter recall includes serial numbers
52600000000 through 52610999999, 52700000000 through 52710999999, and
52800000000 through 52810999999. Please visit www.accu-chek.com
and click your country to take you to the relevant webpage with contact
numbers.
If you have registered your meter, please expect to receive
mailed notification of this recall. Very few malfunctions have been reported however,
if you are not certain if your monitor is affected, please follow with the advice
given on the Accu-chek website as listed above.
Once again, please check the serial numbers on the back of Aviva blood
sugar monitors to see if they might be affected. If the serial number
falls within the range of 52500000000 through 52510999999 or less than 526 10
999 999 it may be affected. To be certain, please check with Roche/Accu-chek at
their website above to clarify these serial numbers.
